All sections of the forms must be completed and returned to the address above. You will need to send a photocopy of your passport, driving license, birth certificate or a tax document from HM Revenue and Customs. If you are a non-EU citizen, we will also need a copy of your work permit confirming you are legally eligible to live and work in the UK. We must have a full and up to date CV detailing your work history and experiences, so if you have not already done so, please forward over your CV by e-mail or enclose a paper copy with your registration forms.
We do retain self-employed chefs as well as those wishing to work via our PAYE scheme. Please complete the appropriate area of the form for self-employed, or the enclosed P46 for PAYE.
When we have received your completed form(s) and ID, a member of the team will contact you to discuss your application and to advise the next steps we must take to get you onto our team.

Should you have any queries about the forms, or your employment with us, please do not hesitate to give us a call. If you do not have access to a printer to re-produce this document, then contact the office by return and a paper copy can be mailed out to you.
